Support available to adults living within the High Peak area provided by Derbyshire Low-Level Support Service.
The service provides intervention and preventative support for adults who maybe struggling to manage day-to-day life at home, they also support individuals who do not meet the threshold for ongoing Adult Social Care or NHS support.
They aim to empower people to maintain their independence, improve their wellbeing, and remain safe and confident in their own homes.
The support on offer includes:

General advice and guidance, including support with housing-related concerns

Support with Benefit checks and form filling

Helping individuals build community connections and access local groups, activities, and services that can reduce isolation and prevent their needs from escalating

One-to-one support to increase confidence, resilience, and independence

Information and referrals for equipment and home adaptations that can help individuals continue living safely at home

Signposting and referrals to other local services that may provide additional support
Their aim is to ensure that anyone who needs support can access the right help at the right time, preventing issues from becoming more significant and helping people to stay independent for longer.
